Dear Sian,
  I have seen this problem - The mismatch between the html form and the
server program seems to result when only half the form information
gets transmistted by Netscape or Internet Explorer. The main reason seems
to be that the form information didn't properly download into the browser.
Have you tried reloading, making sure that you wait long enough for
it download completely, and then click on submit. (If in doubt, scroll
down to the end of the page.). If you still have problems, please do email
me again about it.
